Please download and complete this questionnaire after you have purchased your workshop registration. Take your time to think about your answers. The form requires Adobe Reader version 7 or later. You can download the latest version of Adobe Reader here.

This form is password-protected, and can be used only by registered workshop participants. To download and use the form, you must obtain the password from us. You should have received the password in your confirmation e-mail. If you have not, please send a message requesting the password to workshop@consciouscouplesrecovery.com and we will reply with the questionnaire password.
